Lifeline is a government assistance program that is only available for one phone per household by law. Consumers who willfully make false statements in order to obtain the benefit can be punished by fine or imprisonment or can be barred from the program. Only eligible consumers may enroll in the program. Lifeline benefits are limited to single line of service per residence, consisting of either wire line or wireless service. You may not receive multiple Lifeline discounts. You may apply your Lifeline discount to either one land line or one wireless number, but you cannot have the discount on both. Proof of income or program participation must be returned with a signed Lifeline form in order to obtain service.
